To provide equal and uniform access to all candidates for the distribution of campaign literature, the PDFs below should be used to request labels for distribution of campaign literature. Please be certain that you use the appropriate request form. The forms are for any UUP Election: Chapter, Retiree, Officers and Executive Board, and/or Affiliate.
